Please complete HSL re-opening questionnaire!
Hello Humboldt soccer community!
The Humboldt Soccer League is gearing up to re-open the league for this summer and fall seasons. We are following state guidelines (https://covid19.ca.gov/industry-guidance/) and will follow all Covid regulations. While we encourage everyone to get vaccinated, we will not require proof of vaccination to play.
We have made this questionnaire to gather information to help prepare. Please follow the link below to the questionnaire and submit your responses by Friday June 18th. This form will not collect any personal information.
